WAIVER OF LIABILITY AND INFORMED CONSENT AGREEMENT WARNING! For ALL participants in Impact Academy Inc., by signing this form you give up important legal rights. PLEASE read carefully! Participants must be 18 years old or older OR have a parent/guardian consent.
Description of Risks-In considerations of my participation in Impact Academy Inc., I, the undersigned, state as follows: That I am aware of the possible risks, dangers and hazards associated with the sport and all related activities including the possible risk of severe or fatal injury to myself or others. I freely accept and fully assume all such risks, dangers and hazards; and that I acknowledge my responsibility to discuss my participation in the sport with my physician.
I further agree as follows: TO WAIVE ANY AND ALL CLAIMS arising from my participation in sports that I have or may have in the future against “Impact Academy Inc”, it’s directors, officers, employees, game officials, volunteers and representatives associated with my participation (herein collectively referred to as the “Releasees”); TO RELEASES the Releasees from any and all liability for death or any loss, damage, injury or expense that I may suffer or that my next of kin may suffer arising from my participation in sports due to any cause whatsoever; TO INDEMNIFY the releases from any and all liability for any damage to property of or personal injury to, any third party, arising from my participation on the team; TO AUTHORIZE the releases to use images or video footage taken of myself, in whole or in part, individually or in conjunction with other images and video footage, to be displayed on Impact Academy Inc. website and other official channels, and to be used for media purposes including promotional presentations and marketing campaigns. I waive rights to privacy and compensation, which I may have in connection with use of my image and likeness, including rights to be written copy that may be created in connection with video production, editing and promotion therewith. THAT THIS AGREEMENT is binding upon my heirs, next of kin executors, administrators, assigns and representatives in the event of my death or incapacity; THAT I HAVE READ this agreement AND I UNDERSTAND ITS CONTENT; AND THAT BY SIGNING IT I AM WAIVING CERTAIN LEGAL RIGHTS which I or my heirs, next of kin, executors, administrators or assigns may have against the Releases.

Code of Conduct Form
We aim to provide all participants with a safe, competitive and enjoyable atmosphere for all participants. To ensure this, all participants must agree to abide by the following code of conduct before, during and after all games and practices. Participants are expected to treat everyone with courtesy and respect in all matters including, but not limited to: the context of their sport, gender, place of origin, color of ethnicity, religions, political belief, economic status or ability. This requirement prohibits any form of harassment or discrimination. Participants must abide by the rules and regulations of their sports. All participants are expected to refrain from any action that may endanger themselves, other participants, officials, spectators and staff members or cause damage to equipment and/or facilities used. Verbal or physical abuse of officials or staff by players or spectators will not be tolerated. Attempting to play while under the influence of alcohol or illegal substances is not acceptable. Any participant or team failing to abide by this code of conduct may face immediate or future suspension or expulsion from the game, league or program. No refunds will be issued in these circumstances. I, the undersigned, agree to abide by the code of conduct as described above.
